HV Care is seeking a Care Partner to join our hybrid team servicing Hervey Bay and Maryborough. The role focuses on coordinating aged care services, developing care plans and supporting clients and families within the Wide Bay region.

You will work across Support at Home, CHSP and allied health services, building strong relationships with clients, families and healthcare professionals while ensuring high-quality, compliant care. Hybrid work arrangement is offered to support flexibility.

Competenze

• Experience in aged care, community services or care coordination.
• Strong understanding of person-centred care and client rights.
• Knowledge of the Support at Home program and/or CHSP, or willingness to learn.
• Strong organisational, communication and problem-solving skills.
• Ability to manage multiple client needs and priorities.
• Confidence using electronic client management systems and Microsoft Office.

Mansioni

• Develop, implement and review individualised care plans in consultation with clients and families.
• Coordinate Support at Home and CHSP services, including in-home care and community supports.
• Build trusted relationships with clients, families and healthcare professionals.
• Conduct client home visits, care reviews and wellbeing check-ins.
• Monitor client budgets, service utilisation and care arrangements.
• Support new client onboarding, service agreements and care commencement.
• Identify changes in client needs and coordinate referrals and clinical escalation.
• Maintain documentation and ensure compliance with aged care requirements.
• Develop relationships with local hospitals, GPs and community organisations.
