Churches of Christ in Queensland is seeking a Care Partner for Home Care in the Wide Bay Region (Hervey Bay & Bundaberg). You will coordinate person-centred care, work with registered nurses and clinicians, and help clients stay independent at home through Home Care Packages and CHSP programs.

Ideal candidates bring qualifications in aged care, strong communication, and a compassionate, collaborative approach to support clients and their families.

Competenze

• Certificate III in Individual Support or equivalent required.
• Certificate IV in Ageing Support or related qualification preferred.
• Experience in home or aged care and in multidisciplinary teams.

Mansioni

• Coordinate and review care services to meet client needs and goals.
• Collaborate with RNs and clinical team on assessments and care planning.
• Identify health changes; escalate clinical concerns promptly.
• Incorporate clinical advice into care plans and service agreements.
• Build relationships with clients, families and external providers.
• Develop and update care plans and service agreements.
• Manage client budgets within funding guidelines.
• Identify risks and escalate when required.
• Maintain accurate records and documentation.
• Contribute to a supportive, collaborative team culture.
