Job summary

The Salvation Army Australia is seeking a Case Manager for the Housing Support Program in Darwin, NT. This role provides outreach case management, develops case plans and helps participants access community supports.

You will complete intake, assess needs, maintain records, and advocate with agencies. The position is a maximum term contract until 31 December 2027, paying SCHADS Level 4 in a supportive, inclusive team.

Qualifications

• Degree in Social Work, Mental Health, Welfare or Community Development or equivalent experience.
• Experience assisting people facing homelessness, AOD use, mental health concerns or justice involvement.
• Ability to engage respectfully with diverse communities, including Aboriginal and Torres Strait Islander peoples.
• Strong communication and organisational skills.
• First Aid certification and compliance checks are preferred.

Responsibilities

• Provide case management support within the Housing Support Program and develop case plans.
• Assist participants to attend appointments, build capacity and engage with supports.
• Complete intake and assessments and maintain accurate records.
• Advocate for participants with government and non-government agencies.
• Contribute to a culturally safe, trauma-informed and inclusive service environment.
