Job summary

St Vincent de Paul Society Northern Territory is seeking a Housing Support Worker to provide person-centred assistance within transitional supported accommodation programs in the Darwin area. The role combines case management with housing pathway support, including home visits, advocacy and referrals to internal and external services.

Qualifications

• Relevant qualifications in community services, human services, social work, housing or a related field, or equivalent relevant experience.
• Experience supporting people experiencing homelessness or at risk of homelessness.
• Experience in case management, tenancy support or accommodation services is desirable.
• Experience working with Aboriginal and Torres Strait Islander people and diverse cultures is highly regarded.
• Maintain professional boundaries and protect participant privacy.

Responsibilities

• Build respectful, professional and culturally safe relationships with participants.
• Complete assessments and develop, implement and review case management plans.
• Meet with participants regularly to review progress and barriers.
• Provide advocacy and practical assistance affecting housing stability (income, budgeting, health, education, employment).
• Identify pathways to long-term housing and assist with housing applications and documentation.
• Develop tenancy-readiness skills and manage scheduled home visits and referrals.
• Maintain accurate case notes and program records.
