Wathaurong Aboriginal Co-Operative is seeking an Intensive Case Manager to support Housing First initiatives in Geelong. The role provides intensive case management to Aboriginal community members and families, coordinating access to housing and related services with culturally safe practices.

You will respond to referrals promptly, advocate for services, and work within the Safe Homes and Healing Program to achieve stable housing outcomes in a collaborative team.

Responsibilities

• Provide intensive case management to Aboriginal community members and families accessing Entry Point services.
• Respond to referrals in a timely manner utilizing Housing First Principles of Practice.
• Coordinate and advocate for delivery of services to reduce homelessness risk and support stable housing.
