Job summary

The Intensive Case Management role - Housing First sits within Safe Homes and Healing Program area at Wathaurong Aboriginal Co-Operative as part of the Homelessness team.

The role provides intensive case management to Aboriginal community member’s and families accessing Entry Point services. They will respond to referrals in a timely manner and coordinate access to a range of programs and services to support stable housing.

Qualifications

• Relevant qualifications in community services and social housing.
• Experience working with Aboriginal people experiencing or at risk of homelessness.
• Understanding of issues affecting homelessness in the Aboriginal community.
• Knowledge of Aboriginal culture and commitment to the Aboriginal community.
• Demonstrated case management skills.

Responsibilities

• Provide intensive case management to Aboriginal clients.
• Respond to referrals in a timely manner using Housing First Principles.
• Coordinate and advocate access to programs and services.
• Work outreach and office-based service delivery.
• Ensure culturally safe practices.
• Develop positive relationships with clients and families.
