Job summary

Anglicare WA in partnership with Doors Wide Open and Breakaway Aboriginal Corporation delivers the Housing First Support Services (HFSS) in the Greater Bunbury area. As the Case Manager, HFSS you will provide tailored case management to rough sleepers and assist them to secure and maintain appropriate accommodation and linked supports.

Qualifications

• Tertiary degree in Social Work, Psychology, Social Science, Counselling or demonstrated relevant experience.
• Demonstrated experience working with marginalised and disadvantaged groups.
• Demonstrated case management skills and ability to assess, inform, advise, support and refer clients.
• Demonstrated experience working independently and as part of a team.
• Demonstrated understanding of the issues affecting people experiencing rough sleeping.
• Demonstrated skills in following safety procedures and dynamic risk assessments.
• Ability to work from a strength-based approach and be empathic to the client group.

Responsibilities

• Undertake holistic assessments of client needs, develop case plans and provide practical supports.
• Provide quality case management that is person centred, strengths based, trauma informed and culturally safe.
• Increase clients’ knowledge and skills in budgeting and tenancy maintenance.
• Work in consultation with all clients to provide a range of information and support options to enable them to make choices to enhance their safety and wellbeing
• Advocate for and on behalf of clients as well as provide referral services as needed.
• Encourage clients to engage in the local community and build social networks
