Job summary

Junction Support Services in Wodonga is seeking a Homes First Case Manager to support individuals and families experiencing homelessness to secure and maintain safe, stable housing. You will work within Housing First, trauma-informed and strengths-based practice, developing support plans and advocating for access to health and community services.

You will collaborate with the Homes First team and external providers, maintaining client choice, cultural safety and accurate records.

Qualifications

• Relevant tertiary qualification in Social Work, Welfare or Behavioural Science.
• Experience in trauma-informed, strengths-based case management.
• Knowledge and understanding of Housing First Principles.
• Ability to assess needs in a family context and develop case plans.

Responsibilities

• Deliver person-centred, trauma-informed, strengths-based case management reflecting Housing First.
• Assess client needs and develop, implement and review individualised support plans.
• Support clients to secure and sustain long-term housing through practical assistance and advocacy.
• Build partnerships with clients, families, community services and the Homes First team.
• Maintain accurate case records and comply with Junction policies and governance.
