Junction Support Services invites applications for a Homes First Case Manager in Wangaratta, VIC. You will support individuals and families experiencing homelessness to secure and maintain safe housing through Housing First, trauma-informed, and strengths-based practice.
You will build strong client relationships, assess needs, develop support plans, provide advocacy, and connect clients with health and community services, including after-hours on-call roster as required.

As a Homes First Case Manager at Junction Support Services, you will support individuals and families experiencing homelessness to secure and maintain safe, stable housing through Housing First, trauma-informed and strengths-based practice. You will build strong client relationships, assess needs, develop support plans, provide practical assistance and advocacy, and connect clients with relevant health and community services.
Working alongside the Homes First multidisciplinary team and external providers, you will promote client choice, self-determination and cultural safety, including supporting Aboriginal and Torres Strait Islander clients to maintain connections to culture and community. The role also involves maintaining accurate records, participating in supervision and professional development, and contributing to the after-hours on-call roster as required.
This is a full-time maximum term contract opportunity, based at our Wangarattauntil 30 June 2027.
