Job summary

Junction Support Services in Wangaratta is seeking an Aboriginal Case Manager for the Homes First Program to provide culturally safe, trauma-informed, strengths-based case management. You will help clients set goals, develop support plans and overcome barriers to housing stability, while strengthening connections to culture and community.

The role focuses on building trust, supporting self-determination and creating pathways for Aboriginal people and families to thrive.

Qualifications

• Identification as Aboriginal and/or Torres Strait Islander.
• Qualifications or experience in community services, social work or related fields.
• Experience with Aboriginal children, young people, families or communities.
• Understanding of culturally safe, trauma-informed, strengths-based support.
• Case management skills including assessment, planning and advocacy.
• Knowledge of homelessness and Housing First principles.
• Familiarity with FVISS, CISS and MARAM or willingness to learn.

• Embed Housing First, culturally safe and trauma-informed practice.
• Develop and review individualised support plans addressing housing and wellbeing needs.
• Foster connections to culture, Country and community with local Organisations.

About the Role

As the Homes First Aboriginal Case Manager at Junction Support, you will provide culturally safe, trauma-informed, strengths-based and person-centred case management, ensuring Aboriginal clients have choice, voice and control in the support they receive.

You will work alongside clients to identify their goals, strengths and support needs, develop individualised support plans and provide practical assistance to overcome barriers to housing stability. You will also support clients to strengthen connections with culture, Country and community, and build meaningful pathways towards social and emotional wellbeing.

This role is about much more than housing. It is about building trust, walking alongside people, supporting self determination and creating opportunities for Aboriginal people and families to thrive.

This is a full-time maximum-term contract opportunity until 30 June 2029, based at our Wangaratta location.

What You Can Expect

In the role, you will:

• Support Aboriginal clients to secure, establish and maintain safe, stable and sustainable tenancies through flexible, person-centred case management.
• Embed Housing First, culturally safe, trauma-informed, strengths-based and self-determined practice across all aspects of service delivery.
• Develop and review individualised support plans that address housing, wellbeing, family, parenting and practical living needs.
• Support connection to culture, Country and community, including building strong partnerships with Aboriginal Community Controlled Organisations and local Aboriginal communities.
• Advocate for clients and facilitate connections with health, wellbeing and community services, while proactively identifying and responding to risks that may impact housing stability.
• Maintain accurate case records and reporting, participate in multidisciplinary collaboration, supervision and professional development, and contribute to effective transition and exit planning.

About You

You will bring:

• Identification as Aboriginal and/or Torres Strait Islander.
• Relevant qualifications in Community Services, Social Work, Welfare, Behavioural Science or a related field, and/or relevant experience.
• Experience working with Aboriginal children, young people, families and/or communities.
• Strong understanding of issues affecting Aboriginal communities and experience delivering culturally safe, trauma-informed and strengths-based support.
• Demonstrated case management skills, including assessment, support planning, advocacy and client led goal setting.
• Understanding of homelessness and Housing First principles, with strong communication, collaboration and relationship-building skills.
• Knowledge of family violence information sharing requirements, including FVISS, CISS and MARAM, or willingness to develop this knowledge.
