Lutheran Homes Group in Glyde and Hope Valley is seeking a Care Partner or Clinical Care Partner to support older people to live safely and independently in their own homes. You will work within a person-centred, rights-based framework, coordinating tailored care with families and multidisciplinary teams.

Qualifications

• Minimum Cert IV Community Services or equivalent.
• Experience coordinating care for home care clients is preferred.
• Ability to work autonomously with minimal supervision and uphold age-care standards.

Responsibilities

• Conduct initial assessments and develop person-centred support plans.
• Coordinate services and manage care plans with families and teams.
• Provide skilled in-home care and promote independence.
• Ensure services are delivered with dignity, respect, and cultural safety.
