About the Role
As a Care Partner or Clinical Care Partneryou will support older people to live safely, independently, and with dignity in their own homes. Working within a person-centred, rights-based framework, you will collaborate with consumers, their families, and multidisciplinary teams to coordinate tailored care and support services.
Through a relationship-based approach to care coordination, you will be responsible for care planning, service coordination, and ongoing case management, with a strong focus on wellness, reablement, cultural safety, and empowering consumers to achieve their individual goals and maintain their independence.
This opportunity may suit an Enrolled Nurse (EN), Registered Nurse (RN), or an experienced Home Care Partner with a strong passion for supporting older Australians in the community.
The successful applicant will inherit an established caseload of approximately 40 clients with varying levels of complexity. We are seeking the right candidate to commence in early December 2026, allowing for a minimum four-week handover period to support a smooth transition and ensure continuity of care for clients.
Key Responsibilities
Candidates may be required to undergo a pre-employment medical assessment and must hold a current NDIS Worker Clearance. In line with current health guidelines for the aged care sector, candidates are strongly encouraged to be up to date with all recommended vaccinations.
