Job summary

Mercy Health is seeking a Community Care Partner in the South East region to deliver care management enabling older people to live safely at home. You will work in a rights-based, person-centred framework, coordinating with participants, supporters, and interdisciplinary teams within a Shared Care Team.

The role emphasises care planning, service planning, monitoring and education across streams, with opportunities to collaborate with Clinical Care Partners on complex cases.

Qualifications

• Evidence of or in the process of obtaining: A current NDIS Screening Check.
• Ability to deliver client-centred care and effective case management.
• Strong time management and organisational skills.

Responsibilities

• Care planning including reviewing care plans and budgets.
• Service planning and management with clients and carers.
• Monitor, review and evaluate care with a wellness focus.
• Provide support and education to clients and supporters.
