Job summary

Lutheran Homes Group is seeking a Care Partner or Clinical Care Partner to support older Australians at home, coordinating tailored care and services in a person-centred, rights-based approach. You will work with consumers, families, and multidisciplinary teams to plan and deliver care with a strong emphasis on wellness and independence.

The role may suit Enrolled Nurses, Registered Nurses, or experienced Home Care Partners who are passionate about in-home support.

Qualifications

• Minimum Cert IV Community Services.
• Previous experience coordinating Home and Community aged care is highly regarded.
• Ability to work autonomously and operate with minimal supervision.

Responsibilities

• Conduct initial assessments for new consumers joining the LHG Home Care Service.
• Develop person-centred support plans aligned with notices of decision and approved plans.
• Complete formal reviews of care plans with stakeholders as required.
• Provide skilled, timely care while promoting independence and involvement in decisions.
• Ensure services are consumer-focused with dignity, respect and cultural sensitivity.
• Follow up referrals promptly from various sources.
• Engage in training and development activities.
