Job summary

Alwyndor in Adelaide's southern metro is seeking a dedicated Support at Home Care Partner to develop and manage care plans, budgets, and service delivery with clients, families and healthcare teams.

You will ensure compliance with aged care standards, provide guidance to home care staff, monitor funding, and build strong relationships with providers, government agencies and communities.

Qualifications

• 2+ years’ experience in case management, assessment, planning and evaluation for older people.
• Knowledge of aged care legislation, funding, policy protocols, and duty of care obligations.
• Current driver’s licence and registered, roadworthy vehicle.

Responsibilities

• Develop, maintain and update individualized care plans with clients and families.
• Monitor service delivery and reassess care needs as required.
• Provide advice on service selection, self-management and future care options.
• Act as the primary contact for clients and families, addressing concerns.
• Manage budgets, monitor funding and meet productivity targets.
• Maintain accurate records in line with standards.
• Deliver services according to organisational policies and Aged Care Standards.
