Job summary

UPA Aged Care Solutions in Australia is seeking a Care Partner for Northern Rivers Home Care in Alstonville. You will work with clients to support independence and wellbeing, co-design care plans, and coordinate with the care team to achieve positive outcomes within budget and resource constraints.

Ideal candidates have a tertiary qualification in Aged Care or related field, strong communication, case management experience, knowledge of Home Care Standards, and a current driver licence.

Qualifications

• Tertiary qualification or equivalent in Aged Care, Community Services or health-related discipline.
• Strong communication and interpersonal skills.
• Experience in coordination, case management and care planning.
• Knowledge of Aged Care legislation, Home Care Standards.
• Competency with Microsoft Office and electronic client records.
• Current Driver Licence and Willingness to undergo screening.
• First Aid certificate or willingness to obtain.

Responsibilities

• Co-design care plans based on client goals and needs.
• Support functional independence and wellness in clients' homes.
