Southern Cross Care (SA, NT & VIC) Inc. is seeking a Home Care Coordinator to support person-centred services for clients within the Home Care portfolio. You will be the central contact for clients, carers and domestic staff, ensuring service plans and budgets are followed and issues escalated as needed.

The role requires strong interpersonal and time-management skills, and proficiency with computer systems including My Aged Care. A Diploma of Nursing with AHPRA is desirable but not required.

Qualifikationen

• Onboards and reviews clients, creates budgets and ensures timely service setup.
• Acts as primary contact for clients, carers and staff in the region.
• Advocates for clients to support informed decisions.

• Coordinate service deliveries in home care according to client plans.
• Manage change requests and equipment purchases.
• Coordinate region vendors and complete onboarding admin.
