Healthcare Australia is seeking a people-focused Home Care Coordinator to support clients across the Adelaide area. In this client-facing role you’ll build strong relationships with clients and their families, coordinate services that meet individual needs, and act as a trusted point of contact throughout their Home Care journey.

You’ll spend time meeting with clients in their homes, conducting assessments, developing care plans, and collaborating with Registered Nurses and allied health

Qualifications

• Experience in Home Care, aged care or community services.
• Strong interpersonal and communication skills.
• Ability to travel to client homes and coordinate care.

Responsabilités

• Monitor service delivery through client feedback and care plan reviews.
• Collaborate with RNs and Allied Health professionals to coordinate care.
• Work with internal teams to ensure efficient service delivery.
• Assist with recruitment and matching of suitable care workers.
