genU is recruiting a dedicated Case Manager for its Home Care Packages team in Geelong. You will design and review consumer-directed care plans, coordinate services, and support clients to live independently at home.

Your role involves collaborating with families and health professionals while ensuring plans stay within budget and guidelines. You’ll work in a busy, people-centered environment and help clients set meaningful goals, track progress, and adjust supports as needs evolve.

Qualifications

• Requires tertiary qualification in a human services field and relevant experience in community care or case management.
• Strong communication and interpersonal skills with ability to build relationships with clients and families.
• Proven ability to manage budgets, monitor funding, and ensure service delivery within guidelines.

Responsibilities

• Develop and implement consumer-directed care plans focused on goals and funding.
• Collaborate with clients, families, and networks to reflect individual needs and preferences.
• Regularly review care plans, monitor progress, and adjust supports as needs change.
• Coordinate with allied health professionals and providers, maintaining compliant records.
