Job summary

genU is seeking a caring and motivated Case Manager to join our Support At Home team in Warrnambool. You will develop and implement consumer-directed care plans that reflect individual goals, coordinate services, and work with clients, families and allied health providers to ensure supports are delivered within budget and guidelines.

You will bring a tertiary qualification in a human services field and demonstrated experience in community care and case management, plus strong communication,

Qualifications

• Tertiary qualification in human services or related field.
• Experience in community care and case management.
• Knowledge of ageing and disability and consumer-directed services.
• Ability to adapt to changing needs with a flexible/problem-solving approach.
• Experience with behaviour management strategies.
• Understanding of Commonwealth and State funding programs and aged care standards.
• Excellent communication and interviewing skills; ability to assess clients.
• Ability to work independently and in multidisciplinary teams.
• Strong analytical, budgeting, and computer literacy (MS Office, Carelink+).

Responsibilities

• Develop and implement consumer-directed care plans with measurable outcomes.
• Collaborate with consumers, families and support networks to tailor plans.
• Conduct regular care plan reviews and adjust supports as needs change.
• Coordinate with allied health professionals and maintain accurate records.
