genU in Warrnambool is seeking a caring and motivated Case Manager to join our Support At Home team, helping clients live well and stay independent in their own homes. You’ll work closely with clients, families and allied health providers to develop and review individual care plans that are tailored, practical and focused on each person’s goals.

You’ll coordinate services, monitor changing needs and ensure supports are delivered within budget and compliant.

Formación

• Tertiary qualification in a human services field – this may include Nursing, Social Work, Welfare, Disability, Case Management, Registered Nurse or Allied Health.
• Demonstrated experience in community care, particularly care/case management.
• Understanding of the social, psychological and physical aspects of ageing and disability including the role of carers and a commitment to consumer directed services.
• Flexibility and innovation in responding to changing needs.
• Knowledge, skill and experience in delivering behaviour management strategies.
• NDIS Worker Screening Check and Orientation Module Certificate (we’ll help guide you through this).
• Strong understanding of Commonwealth and State funding programs, including Aged Care Quality and Safety Standards.
• Excellent communication and interpersonal skills, with experience in interviewing and conducting client assessments.
• Ability to work independently and collaboratively in a fast-paced, multidisciplinary environment.
• Strong analytical, budgeting, and problem-solving skills with advanced computer literacy (MS Office and Carelink+ desirable).

Responsabilidades

• Develop and implement comprehensive, consumer-directed care plans focused on goal outcomes, funding requirements, and measurable results.
• Collaborate with consumers, families, and support networks to ensure care plans reflect individual needs and preferences.
• Conduct regular care plan reviews, monitor progress, and adjust supports in response to changing client needs.
• Coordinate with allied health professionals and service providers while maintaining accurate records and compliance with Support At Home Packages guidelines.
