Regis Home Care is seeking a Client Services Manager to lead care planning and service delivery for clients in the Melbourne southeast region. You will develop person-centred care plans, assess needs, and coordinate with clinicians and Home Support Workers to ensure safe, dignified home-based care.

Qualifications

• Solid time management to meet client needs and schedules.
• Must hold a current driver’s license.

Responsibilities

• Develop care and support plans aligned with client needs and aspirations.
• Conduct assessments and create person-centred care plans to foster independence and wellbeing.
• Assist clients and families to use Home Care Package funding within guidelines.
• Build partnerships with clinical experts, allied health professionals and Home Support Workers.
• Travel to visit clients; company car provided where required.
