Disability Support Worker
Position Overview

The Disability Support Worker works under the supervision of the Registered Nurse and/or Manager-Client Services Division at Im With You. This role involves providing comprehensive, personalised support to NDIS participants and their families through individual and group activities, assisting with daily living tasks, implementing positive behaviour support plans, facilitating social activities, and supporting learning opportunities in both home and community environments. Working within Im With You's unique model of care, this position utilises digital client profiles, personalised training, and a collaborative healthcare approach to deliver exceptional person-centred support that promotes independence and community participation.

Key Responsibilities
  • Provide day-to-day support to NDIS participants and their families as per roster and individual support plans
  • Support participants in age-appropriate activities and tasks, ensuring safe planning and implementation
  • Encourage active participation from all clients in accordance with their individual support plans
  • Assist clients with all aspects of daily living in home environments including personal care, domestic tasks, and community access
  • Implement positive behaviour support plans under professional guidance from registered nurses and allied health professionals
  • Support participants to access community activities and outings with direct care plans
  • Ensure all activities and outings are conducted according to scheduled times and appointments
  • Deliver person-centred care that respects client preferences, choices, and individual goals
  • Build trusting, professional relationships with clients and their families
  • Support clients' independence and capacity building in line with NDIS goals
  • Adapt support approaches based on individual client needs and preferences
Care Planning and Coordination
  • Participate in the development of individual support plans
  • Monitor, evaluate, and review client progress and support plan effectiveness
  • Assist in coordinating day-to-day activities with other staff working with NDIS participants
  • Work collaboratively within a multi-disciplinary team including registered nurses, physiotherapists, occupational therapists, and other healthcare professionals
  • Coordinate activities with clients to ensure seamless service delivery
  • Communicate effectively with clinical leadership regarding client needs and progress
  • Support implementation of recommendations from allied health professionals
  • Participate in client review meetings and case conferences as required
Compliance and Safety
  • Work in accordance with medication administration policies and procedures
  • Follow all work health and safety procedures
  • Adhere to client rights policies including privacy and confidentiality
  • Ensure all tasks are completed in line with organisational policies and procedures
  • Maintain safe and hygienic environments at all times

  • Document all tasks and activities accurately for assessment and review purposes
  • Update reports in a timely manner as per organisational requirements

  • Ensure all communication is professional and polite with internal and external stakeholders
  • Complete shift notes and progress reports according to organisational standards
  • Document client observations, incidents, and changes in condition promptly
  • Maintain confidentiality in all written and electronic communications

  • Certificate IV in Disability Support or higher qualification

  • Additional relevant certifications including:
    • Manual handling training
    • Medication administration training
    • Epilepsy awareness and management
