Alternate Care in Cairns is seeking a full-time Foster Care Case Manager to support Foster and Kinship Carers and the children in their care. This on-site role focuses on safety, stability and positive outcomes through collaborative case management and strong stakeholder engagement.

Qualifications

• Mandatory qualification – Cert IV or higher in a relevant human services discipline.
• Knowledge and/or experience in working within the child protection system is desirable.
• Strong case management and administrative skills including commitment to monthly reports and minimum monthly visits Foster or Kinship Carers on site.
• Excellent written and verbal communication across a wide range of stakeholders including Department of Child Safety, Education, Health and natural family, and attending care team meetings.
• Willingness to comply with organisation policies and procedures and understand trauma-informed practice.

Responsabilités

• Provide day-to-day support to Foster and Kinship Carers in caring for children and young people.
• Undertake all aspects of case management including assessment, planning, implementation and review of therapeutic strategies and support plans.
• Coordinate care teams and maintain effective relationships with key stakeholders.
• Develop and implement safety plans to support placement stability and wellbeing.
• Conduct a minimum of monthly home visits with Foster or Kinship Carers and maintain accurate case notes and documentation.
• Support children and young people to achieve positive outcomes, including reunification and transition to independent living where appropriate.
• Participate in care team and stakeholder meetings to ensure placement needs are met and safety and wellbeing are upheld.
