NSW Health in Sydney is seeking a compassionate Registered Nurse to join the Medical/Stroke Ward on a temporary full-time or part-time basis up to 38 hours per week, through December 2027. You will provide high-quality nursing care to patients with stroke, cardiac and general medical conditions within a multidisciplinary team.

You will engage in discharge planning, support patient and family education, and contribute to positive patient outcomes while upholding evidence-based practice and safety

Formación

• Current Authority to Practice as a Registered Nurse with AHPRA.
• Recent experience as a Registered Nurse in a relevant environment.
• Strong communication with patients, carers and the health care team.

Responsabilidades

• Deliver high-quality nursing care for stroke, cardiac and general medical patients.
• Coordinate continuum of care from acute treatment to rehabilitation and community management.
• Engage patients and families in shared decision-making and education.
• Lead discharge planning and rehabilitation initiatives from admission.
