The role is offered as full-time or part-time, up to 38 hours per week, with remuneration described as $37.83 - $41.10 per hour plus 12% superannuation and salary packaging.

Qualifikationen

• Current Authority to Practice as an Enrolled Nurse (Division 2) with AHPRA.
• Recent experience as a Registered Nurse in an appropriate specialty.
• Ability to effectively communicate with patients/consumers, carers and the multidisciplinary health care team.

Aufgaben

• Provide care to patients with stroke, cardiac and general medical conditions within a multidisciplinary team.
• Support seamless care across acute, rehabilitation and community services through collaboration with hospitals and community providers.
• Assist in patient and family involvement in care planning, education and discharge planning to optimise recovery.
• Participate in discharge planning and rehabilitation from admission to post-discharge care.
• Commit to clinical excellence, education, research and evidence-based practice in the ward.
