Job summary

GOC Care in Brisbane is seeking an experienced Support at Home Care Partner to coordinate services under the Support at Home Program and CHSP. You will work with participants and families, manage care plans, conduct reviews, and ensure services meet quality standards.

Key requirements include Aged Care Case Management experience, My Aged Care/CHSP knowledge, strong communication skills, and a genuine passion for helping older people maintain independence.

Qualifications

• Experience in Aged Care Case Management.
• Knowledge of My Aged Care, CHSP and HCP.
• Familiarity with Aged Care Quality Standards.
• Current First Aid & CPR certification.

Responsibilities

• Manage client cases, including assessments, care planning, service implementation and reviews.
• Build relationships with clients and families to ensure safe, high‑quality, person‑centred care.
• Monitor client budgets and unspent funds in line with program guidelines.
• Maintain accurate client records and documentation per compliance standards.
• Liaise with internal teams and external health professionals to deliver holistic care.
• Ensure care aligns with Aged Care Quality Standards.
