Job summary

Evolve Housing is seeking a Case Manager for the Support Pathways program. The role involves managing a caseload, conducting field visits, and coordinating with healthcare, housing and social support networks to achieve positive tenancy outcomes.

You will build collaborative support plans, engage with residents, document progress, and participate in community housing initiatives. A not-for-profit, social housing provider offering flexible hours and career development.

Qualifications

• Degree qualified in Human & Social sciences (e.g. social work, mental health, community development).
• Minimum 3 years hands-on case management experience with diverse, complex needs.
• Knowledge of homelessness issues and trauma-informed approaches.
• Proven ability to engage with clients and develop comprehensive support plans.
• Proficiency with SRS, Office 365 and Excel; ability to learn new software.

Responsibilities

• Oversee a designated caseload from referral to exit.
• Conduct daily face-to-face client meetings and respond to emergencies.
• Develop and implement collaborative individual support plans.
• Engage with external support partners and coordinate weekly follow-ups.
• Attend relevant meetings and promote service programs internally and to clients.
• Maintain up-to-date documentation in the database and other tools.
