Qualifications

• Demonstrated experience delivering support and crisis intervention within a Case Management model.
• Knowledge of housing programs including Housing First approaches.
• Skills in assessment, monitoring and reviewing client plans.

Responsibilities

• Proactively engage people sleeping rough in places they stay or agreed locations.
• Respond to immediate needs including safety, shelter, food, warmth, and medical support.
• Develop and review client-led, goal-focused case plans.
