Sacred Heart Mission is seeking a Supportive Housing Case Manager to join the Homes First Inner South team in Melbourne. You will deliver culturally safe, three-year flexible support packages to help participants sustain tenancies and access local community services.

You’ll work long-term with a small caseload, focusing on individualised goal planning, assertive outreach, and coordination with other agencies.

Qualificações

• An appropriate degree qualification relevant to community services.
• Experience in case management and navigating complex service systems.
• A passion for sustaining tenancies and ending homelessness in Melbourne.
• Ability to adapt to participants' needs and collaborate with different services.
• Demonstrated de-escalation and crisis response capabilities.

Responsabilidades

• Deliver case management and service coordination to participants.
• Build relationships with participants and link them to local community resources.
• Support participants to move into, then sustain, tenancies with goal-based planning.
• Conduct regular outreach and maintain a small caseload focused on outcomes.
