Future Focus Group Pty Ltd in Shepparton is seeking an experienced NDIS Support Coordinator to join the Goulburn Valley team. You will deliver high-quality, person-centred supports to participants and help implement their plans.

The role offers hybrid flexibility, autonomy to manage caseloads, and ongoing professional development within a national organisation. You will work with a supportive leadership team and contribute to better outcomes for participants.

Qualifications

• Qualification in a related discipline (minimum Diploma in Community Services, Social Work, Mental Health, Psychology, Allied Health or equivalent).
• Experience providing NDIS Support Coordination or complex case management.
• Current Driver’s Licence and reliable vehicle.
• Ability to obtain or hold: NDIS Worker Screening Check, WWCC, and Police Check.

Responsibilities

• Deliver high-quality, person-centred Support Coordination to NDIS participants in the greater Bendigo region.
• Guide participants to implement NDIS plans, coordinate supports and build capacity to engage with health, housing, justice and mental health sectors.
• Manage caseload with independence, professionalism and compassion.
