GV Complete Care Support Services Pty Ltd in Goulburn Valley, VIC is seeking an experienced NDIS Support Coordinator. You will manage your own participant caseload, implement NDIS plans, and coordinate supports to enable client choice and outcomes.

The role emphasises independent work within a supportive team, with 20–30 hours per week. A strong background in NDIS and psychosocial supports is preferred, with established documentation and advocacy skills.

Qualifications

  • At least 2 years' experience in NDIS Support Coordination.
  • Strong knowledge of the NDIS, plan implementation, funding and support pathways.
  • Experience supporting participants with complex or changing needs.
  • Experience in Psychosocial Recovery Coaching and/or supporting people with psychosocial disability is highly regarded.
  • Understanding of Behaviour Support Plans, positive behaviour support and regulated restrictive practices.
  • Strong advocacy, problem-solving and communication skills.
  • Excellent organisation and case management skills.
  • Strong report writing and documentation skills.
  • Ability to manage own caseload, competing priorities and deadlines.
  • Confidence to work independently while contributing to a team.
  • Understanding of participant rights, choice and control, supported decision-making and capacity building.
  • Understanding of professional boundaries, safeguarding and conflicts of interest.
  • Relevant qualifications in Community Services, Disability, Social Work, Mental Health, Counselling or related discipline is highly regarded.

Responsibilities

  • Prepare high-quality reports, documentation and supporting evidence.
  • Monitor reassessments, reporting requirements and important participant actions.
  • Maintain accurate progress notes, participant records and claimable activity.
  • Identify safeguarding concerns and escalating risks as needed.

GV Complete Care is a locally owned and operated, registered NDIS provider delivering person-centred disability supports throughout the Goulburn Valley.

We are growing our team and are seeking an experienced NDIS Support Coordinator who is passionate about supporting participants to achieve their goals, exercise choice and control, and access the supports they need.

This is an excellent opportunity for an experienced, local Support Coordinator who enjoys working independently while being part of a supportive and collaborative team.

About the Role

As a Support Coordinator, you will manage your own participant caseload and work alongside participants, families and their support networks to implement NDIS plans and achieve meaningful outcomes.

You will:

Manage an allocated participant caseload and provide responsive, person-centred Support Coordination.

Support participants to understand and implement their NDIS plans, funding and available supports.

Advocate for participants and assist them to exercise choice and control.

Coordinate NDIS-funded, mainstream, community and informal supports.

Monitor plan utilisation, funding and participant needs, identifying gaps or emerging concerns.

Build strong relationships with participants, families, providers and other professionals.

Work collaboratively with allied health professionals, Behaviour Support Practitioners and other providers.

Prepare high-quality reports, documentation and supporting evidence.

Monitor reassessments, reporting requirements and important participant actions.

Maintain accurate progress notes, participant records and claimable activity.

Identify and appropriately escalating safeguarding concerns, service delivery risks and significant changes in participant circumstances.

About You

We are looking for someone who is proactive, organised, confident and genuinely participant-focused.

You will ideally have:

At least 2 years' experience in NDIS Support Coordination.

Strong knowledge of the NDIS, plan implementation, funding and support pathways.

Experience supporting participants with complex or changing needs.

Experience in Psychosocial Recovery Coaching and/or supporting people with psychosocial disability will be highly regarded.

Understanding of Behaviour Support Plans, positive behaviour support and regulated restrictive practices.

Strong advocacy, problem-solving and communication skills.

Relevant qualifications in Community Services, Disability, Social Work, Mental Health, Counselling or a related discipline will be highly regarded.
