We are seeking a qualified and experienced Support Coordinator to join our Melbourne East team. This role offers hybrid flexibility, the autonomy to manage your own caseload, and the backing of a national organisation with strong local leadership. The Central Office for this regions team days is in Eastland, Ringwood.
Reporting to the East Business Unit Manager, you will deliver high-quality, person-centred Support Coordination to NDIS participants throughout the Eastern Suburbs of Melbourne. You’ll guide participants to implement their NDIS plans, coordinate supports, and build their capacity to engage confidently with the broader service system including health, housing, justice, and mental health sectors.You’ll thrive in a role that values commitment and compassion, and trusts you to lead your work.
You are a capable, compassionate professional who combines deep sector knowledge with sound judgment and initiative. You have a background in disability, community, or mental health services, and you take pride in evidence-based, ethical practice.Essential:Qualification in a related discipline (minimum Diploma in Community Services, Social Work, Mental Health, Psychology, Allied Health or equivalent)
