Future Focus Group Pty Ltd in the Hunter Valley region is seeking an experienced NDIS Support Coordinator to provide person‑centred coordination, plan implementation support and system navigation. The role offers hybrid work, autonomy to manage caseloads, and opportunities for professional development within a national provider.

You will work with participants to connect supports across health, housing, and community services, with a focus on ethical practice and outcome‑driven care.

Qualifications

• Qualification in a related discipline (minimum Diploma in Community Services, Social Work, Mental Health, Psychology, Allied Health or equivalent).
• Experience providing NDIS Support Coordination or complex case management.
• Live in the Hunter Valley Region of NSW.
• Current Driver’s Licence and reliable vehicle.
• Ability to obtain or hold: NDIS Worker Screening Check, WWCC, and Police Check.

Responsibilities

• Deliver high‑quality, person‑centred Support Coordination to NDIS participants in the Hunter Valley.
• Guide participants to implement their NDIS plans and coordinate supports.
• Build participants' capacity to engage with health, housing, justice and mental health services.
• Manage your own caseload with autonomy and accountability.

About the Role

Reporting to the Hunter Valley Business Unit Manager, you will deliver high-quality, person-centred Support Coordination to NDIS participants throughout the Hunter Valley region. You’ll guide participants to implement their NDIS plans, coordinate supports, and build their capacity to engage confidently with the broader service system including health, housing, justice, and mental health sectors.

About You

You are a capable, compassionate professional who combines deep sector knowledge with sound judgment and initiative. You have a background in disability, community, or mental health services, and you take pride in evidence-based, ethical practice.

Essential:

• Qualification in a related discipline (minimum Diploma in Community Services, Social Work, Mental Health, Psychology, Allied Health or equivalent)
• Experience providing NDIS Support Coordination or complex case management
• Live in the Hunter Valley Region of NSW
• Current Driver’s Licence and reliable vehicle

Highly Regarded:
