Reporting to the Hunter Valley Business Unit Manager, you will deliver high-quality, person-centred Support Coordination to NDIS participants throughout the Hunter Valley region. You’ll guide participants to implement their NDIS plans, coordinate supports, and build their capacity to engage confidently with the broader service system including health, housing, justice, and mental health sectors.
