Job summary

Sydney Region Aboriginal Corporation (SRAC) in Penrith is seeking an Elder Care Support Coordinator (Aboriginal Identified Position) to assist Elders and their families in navigating local aged care services and entitlements. The role focuses on advocacy, planning, and culturally safe processes to ensure appropriate care pathways.

The position requires understanding of issues affecting older Aboriginal people, a commitment to trauma-informed practice, and appointment of Aboriginality as required

Qualifications

• Aboriginality is a genuine qualification for this position.
• Understanding issues affecting older Aboriginal people in the community.
• Certificate in Community Services or Aged Care, or equivalent experience.
• Strong verbal and written communication skills; experience recording case notes.
• Ability to work with people with complex needs while maintaining professional boundaries.
• Familiar with local Aboriginal communities in Nepean/Blue Mountains and Greater Western Sydney.
• Must have or be willing to obtain National Police Check and NSW Working with Children Check.
• Must hold a NSW unrestricted driver licence.

Responsibilities

• Provide advocacy to ensure culturally safe plan assessments.
• Assist clients to understand aged care entitlements, including My Aged Care.
• Support preparation for plan reviews and outcomes for Elders and families.
• Navigate access to local aged care support services.
