Job summary

The role partners with the Elder Care Support Coordinator to assess local demand, map existing services, address gaps, and advocate for clients.

It requires commitment to Aboriginal health, travel to remote locations, and strong communication skills.

Qualifications

• Experience in health or care sector with older Aboriginal and Torres Strait Islander people.
• Knowledge of aged care landscape or willingness to learn.
• Excellent record management, data collection and reporting.
• Strong communication and interpersonal skills.
• Willingness and ability to travel to remote locations.
• Commitment to Aboriginal health and community-controlled care.

Responsibilities

• Connect with local Elders and families to raise awareness of aged care entitlements.
• Collaborate with the Elder Care Support Coordinator to assess local demand.
• Identify existing aged care services and gaps; advocate for clients as required.
• Support community-led engagement and education on entitlements.
