Job summary

Grand Pacific Health in Far West NSW is seeking an Aboriginal Care Navigator to provide culturally safe support for Aboriginal and Torres Strait Islander people, families and carers. The role strengthens connections with local communities, facilitates warm referrals and collaborates with service providers to improve health and wellbeing outcomes.

Based in the Far West, we offer both full-time and part-time permanent roles.

Qualifications

• Identify as an Aboriginal and/or Torres Strait Islander person and be an active member of an Aboriginal community.
• Relevant tertiary qualification and/or extensive experience in Aboriginal health, alcohol and other drugs, mental health, community services or social sciences.
• Demonstrated ability to provide culturally appropriate care navigation and referral support, with strong interpersonal, collaboration, organisational and digital skills.

Responsibilities

• Provide culturally appropriate care navigation, information and practical support to Aboriginal and Torres Strait Islander people, families and carers.
• Build warm referral pathways and relationships with Aboriginal communities, Aboriginal Community Controlled Organisations and other local and regional services.
• Support outreach, community engagement, advocacy and group activities that improve access, cultural safety, mental health literacy and social and emotional wellbeing.
