Job summary

Connect Home Care in the Central Coast Region is seeking 6 experienced female disability support workers to join our team. You will support participants in Supported Independent Living (SIL) and in community settings, delivering consistent, safe, and meaningful care.

Responsibilities include delivering person-centred supports, assisting with personal care, enabling community access, and documenting shift notes and incidents.

Qualifications

• Experience with challenging behaviours/high complex needs.
• Understanding of positive behaviour support.
• Ability to stay calm in high-pressure situations.
• Strong communication and teamwork skills.
• NDIS Worker Screening Check and WWCC/First Aid required.

Responsibilities

• Deliver person-centred supports aligned with care plans.
• Support personal care and daily living tasks.
• Assist with community access and engagement.
• Complete shift notes and incident reporting.
• Collaborate with families and allied health teams.
