Job summary

• Assist with daily living, personal care, meal preparation and household tasks where required.
• Support participants to access the community, social activities, appointments and recreational activities.
• Encourage independence, choice, dignity and participation.
• Build positive and respectful relationships with participants and their support networks.
• Complete required documentation and follow support plans, safety procedures and workplace policies
• Follow individual support plans and organisational procedures.
• Complete progress notes and required documentation.
• Work collaboratively with families, coordinators and the wider support team.
