Care Connector (Holistic Care Solutions) in Coffs Harbour seeks compassionate Disability Support Workers to provide high-quality, person-centred support to individuals living with disability. You will help clients achieve goals, build independence, and stay active in the community, with shifts across 9 am–3 pm, 3 pm–9 pm, and 9 pm–6:30 am sleepovers.

Up to 30 hours per week, flexible to fit availability. Applicants should have Certificate III/IV in Individual Support and experience with complex

Qualifications

• Certificate III/IV in Individual Support (or equivalent experience).
• Experience supporting participants with complex needs (desirable).
• Current Driver's Licence and reliable vehicle with Comprehensive Insurance.
• Current NDIS Worker Screening Clearance.
• Current Working with Children Check.
• Current National Police Check.
• Current First Aid & CPR Certificate.

Responsabilités

• Deliver safe, person-centred care to participants with complex needs.
• Assist with personal care, daily living and community access.
• Follow care plans and positive behaviour support strategies.
• Monitor and report changes in client condition.
• Complete documentation (progress notes, incident reports).
• Build strong, professional and respectful relationships with clients.
