Job summary

Darling Downs Health in Queensland seeks a Coordinator Home Care to guide care planning, support a dedicated team and build strong links with clients, families and health professionals across the South Burnett region.

You will coordinate personalised support, foster partnerships with carers and providers, and lead staff to deliver safe, high-quality services while driving service improvements that benefit client outcomes.

Qualifications

• Experience coordinating community-based or aged care services, including care planning and case management
• Strong communication skills and the ability to work collaboratively with clients, carers and multidisciplinary teams
• Experience leading staff and managing priorities in a dynamic service environment
• Confidence using digital systems, databases and administrative processes to support quality care

Job description

Lead community-based aged care services in Kingaroy and help older people live safely, independently and with confidence. As Coordinator Home Care, you’ll guide care planning, support a dedicated team, and build strong connections with clients, families and health professionals while making a meaningful difference across the South Burnett community.

What your day could look like

As Coordinator Home Care, you'll help older people remain independent and supported at home. Working with clients, families and service providers, you'll coordinate quality care and lead a team focused on positive outcomes across the community.

Every day, you'll contribute by:

• Coordinating personalised support that empowers clients to achieve their goals and make informed choices
• Building strong partnerships with carers, health professionals and community services to deliver seamless care
• Leading and supporting home care staff to maintain safe, responsive and high-quality services
• Driving service improvements, compliance and resource management that strengthen client outcomes

You'll stand out from the crowd if you have a Certificate III or higher in Individual Support (Ageing) or similar and/or experience working with aged care quality standards.
