Job summary

Darling Downs Health is seeking a Coordinator Home Care in Inglewood to help older people remain independent and connected at home. You will lead a community-based team, coordinate person-centred services and build strong partnerships with clients and health professionals.

You’ll bring passion for compassionate care, strong leadership, and a willingness to travel as required in a rural and regional setting.

Qualifications

  • Experience coordinating community, aged care or home-based services.
  • Strong communication and stakeholder relationship skills.
  • Current C class driver's licence and willingness to travel.

  • Coordinating person-centred home care services that support client goals, wellbeing and independence.
  • Building strong partnerships with clients, families, health professionals and community organisations to deliver seamless care.
  • Leading and supporting a team to provide safe, responsive and high-quality services.
  • Identifying opportunities to improve service delivery, quality outcomes and client experiences.

  • Experience coordinating community, aged care or home‑based support services.
  • Strong communication and relationship‑building skills across diverse stakeholders.
  • Demonstrated ability to lead, support and develop team members.
  • Current C class driver's licence and willingness to travel as required.
