Darling Downs Health in Toowoomba is seeking a Clinical Coordinator - Senior to lead and coordinate specialist geriatric, rehabilitation and stroke services. You will shape care pathways, guide a multidisciplinary team and drive service improvements in our regional health service.

You will coordinate patient-centred care, mentor staff, and collaborate across services to deliver optimal outcomes for patients and families while supporting the regional health system’s goals and values.

Qualifications

• Allied health professional qualification with current registration or membership.
• Strong clinical experience in rehabilitation, stroke, geriatric or related settings.
• Proven ability to lead, influence and support multidisciplinary teams.
• Excellent communication, stakeholder engagement and service improvement skills.
• Desirable postgraduate study in a relevant clinical or leadership discipline.

Lead and coordinate high-quality geriatric, rehabilitation and stroke services in Toowoomba. As a Clinical Coordinator - Senior, you'll play a key role in improving health outcomes for people accessing specialist rehabilitation, stroke and geriatric services. From coordinating patient-centred care to supporting service improvement and clinical excellence, you'll work with a skilled multidisciplinary team to deliver meaningful outcomes for patients, families and the wider community.
