Darling Downs Hospital & Health Service – Inglewood QLD is seeking a Coordinator Home Care to help older people maintain independence and wellbeing while staying connected to their community.

You will coordinate personalised support, guide a dedicated team and collaborate with clients, families and health professionals to deliver high-quality services across the region.

Qualifications

• Experience coordinating community, aged care or home-based support services.
• Strong communication and relationship-building with diverse stakeholders.
• Ability to lead, support and develop team members.
• Current C class driver’s licence and willingness to travel as required.

As Coordinator Home Care, you'll play an important role in helping older people maintain their independence and wellbeing while remaining connected to their community. From coordinating personalised support to guiding a dedicated team, you'll contribute to positive outcomes that make a genuine difference every day.

Every day, you'll contribute by:

• Coordinating person-centred home care services that support client goals, wellbeing and independence.
• Building strong partnerships with clients, families, health professionals and community organisations to deliver seamless care.
• Leading and supporting a team to provide safe, responsive and high-quality services.
• Identifying opportunities to improve service delivery, quality outcomes and client experiences.

• Experience coordinating community, aged care or home-based support services.

• Demonstrated ability to lead, support and develop team members.
• Current C class driver's licence and willingness to travel as required.
