Job summary

Darling Downs Health is seeking a Coordinator Home Care to help ageing clients remain independent at home. You will coordinate personalised support, build partnerships with carers and health professionals, and lead home care staff to deliver safe, high-quality services.

You will drive service improvements and ensure compliance with aged care standards while supporting clients to achieve their goals and maintain dignity and independence in their home environment.

Qualifications

  • Experience coordinating community-based or aged care services, including care planning and case management.
  • Strong communication skills and the ability to work collaboratively with clients, carers and multidisciplinary teams.
  • Experience leading staff and managing priorities in a dynamic service environment.
  • Knowledge of aged care quality standards is advantageous.
