Job summary

Darling Downs Health is seeking a Coordinator Home Care to help older people stay independent at home. You will coordinate personalised support, build partnerships with carers and health professionals, and lead a team to deliver safe, high‑quality community care across the region.

You will drive improvements and use digital systems to support care planning, while balancing resources and compliance to achieve positive client outcomes. Experience in aged care and care planning is essential.

Qualifications

• Experience coordinating community-based or aged care services, including care planning and case management.
• Strong communication skills and the ability to work collaboratively with clients, carers and multidisciplinary teams.
• Experience leading staff and managing priorities in a dynamic service environment.
• Confidence using digital systems, databases and administrative processes to support quality care.

Job description

As Coordinator Home Care, you'll help older people remain independent and supported at home. Working with clients, families and service providers, you'll coordinate quality care and lead a team focused on positive outcomes across the community.

• Coordinating personalised support that empowers clients to achieve their goals and make informed choices
• Building strong partnerships with carers, health professionals and community services to deliver seamless care
• Leading and supporting home care staff to maintain safe, responsive and high-quality services
• Driving service improvements, compliance and resource management that strengthen client outcomes

You'll stand out from the crowd if you have a Certificate III or higher in Individual Support (Ageing) or similar and/or experience working with aged care quality standards.
