Job summary

Darling Downs Hospital & Health Service in Toowoomba (QLD) is seeking a Coordinator Home Care to lead high-quality community care services. You will coordinate personalised care plans, guide a team of home care workers and collaborate with clients, families and health partners to deliver safe, dignified outcomes at home.

You will bring experience in aged care or client-centred support, strong care-planning and digital-record keeping, plus the ability to mentor staff and adapt to changing client

• Experience coordinating community, aged care or client-centred support services.
• Strong skills in care planning, stakeholder engagement and responding to changing client needs.
• Experience leading, mentoring or supervising staff in a service environment.
• Confidence using digital systems, maintaining accurate records and managing competing priorities.
