True Ability in Melbourne is hiring experienced Support Workers for high-intensity and complex care. You will support a participant with complex needs, follow care plans, and monitor wellbeing while maintaining accurate records.

The role requires Certificate III/IV in Individual Support and experience with severe dysphagia, choking first aid, suctioning, and NJ feeding tube medications. Flexible shifts include night, morning and day slots.

Qualifications

• Certificate III/IV in Individual Support required.
• Experience supporting participants with complex or high-intensity needs.
• Experience with severe dysphagia is desirable.
• Choking First Aid competency is required.
• Experience with suctioning procedures and NJ feeding tube medications.

Responsibilities

• Provide person-centred support to a participant with complex care needs.
• Implement support strategies per care and risk management plans.
• Monitor wellbeing and respond to changes in condition.
• Assist with daily living activities while promoting independence.
• Administer medications per policies and procedures.
• Maintain accurate documentation and progress notes.
• Create a safe, respectful environment and work with families and health professionals.
• Follow complex care plans and clinical instructions; stay alert during shifts.
• Communicate clearly and document effectively; work across weekdays, weekends and nights.
