A leading home care provider in Western Australia is seeking a compassionate Care Partner to support older Australians and people living with disability. The role involves designing care plans, managing a caseload, and working closely with clients and healthcare professionals. Ideal candidates will have a Certificate III/IV in Aged Care or relevant qualifications, and enjoy relationship-based care. Competitive pay and career development opportunities available.

Qualificações

• Experience in care coordination, case management, or service delivery within aged care, disability, or community care.
• Current Australian driver’s licence and reliable vehicle.
• Current First Aid and CPR certification.
• National Police Check and Working With Children Check (or willingness to obtain).
• Commitment to ongoing learning and person-centred practice.
