Home Care Assistance Sunshine Coast is seeking a compassionate Care Partner to support older Australians to stay safe, healthy and connected at home. You will manage a portfolio of Home Care Package clients across the Sunshine Coast region and work closely with clients, families, care workers and health professionals to coordinate services.

Competenze

• Minimum Certificate IV in Community Services or related field and/or 1-2 years' experience as a Care Partner in aged care
• ENs are welcome to apply
• Current national police check (or willingness to obtain)
• Working rights in Australia

Mansioni

• Conduct client onboarding, assessments and care planning.
• Complete regular care plan reviews and monitor changing needs.
• Coordinate services and supports with clients and families.
• Identify risks and escalate clinical concerns as needed.
• Collaborate with allied health providers, GPs, hospitals and community services.
• Monitor client budgets and support informed decision-making.
• Maintain accurate documentation and compliance with aged care requirements.
• Support positive client outcomes through proactive case management and relationship building.
